Orthognathic surgery is an operation performed by an oral surgeon. It corrects irregularities of the bones of the face, particularly the jaws, that cause overbites, underbites and crossbites and other facial dysfunction. These irregularities can make it hard for people to sleep (moderate to severe sleep apnea due to a decrease in volume of the oral pharyngeal airway), chew (a malocclusion) and talk.
Orthognathic surgery rectifies these difficulties and, in combination with orthodontic therapy, will enhance the general look and function of the face. The surgery is performed on adults and teens after they’ve finished growing.

The treatment starts off with evaluations by an orthodontist and an oral surgeon. The doctors take X-rays, models of the teeth, photos and facial measurements. They analyze the data, and a proposed treatment strategy is designed.
The braces are already in place when the patient is brought into the operating room and put under general anesthesia. The oral surgeon uses small titanium screws and plates to connect the shifted sections to the jaws. More simply, the upper jaw (maxilla) and lower jaw (mandible) are placed in their anatomically correct positions. This then gives the patient a proper bite, opening of the airway and facial balance.
The patient goes back to the orthodontist six to eight weeks after the surgery, which marks the beginning of the post-operative orthodontic phase to fine-tune and perfect the final obstruction.
